Q: What does Soltis Senior Care Connections do?
A: SSCC provides a variety of services that assist older adults with the challenges of aging. Following a comprehensive assessment, an individual plan of care is recommended that prioritizes a client's needs, wants and interests. We serve as professional liaisons between client and their service providers and can monitor and assist in coordinating all involved in the plan of care.

Q: Where and when does the assessment take place?
A: Usually in the client's home at a time convenient for the client and his or her family.

Q: What type of referrals are provided?
A: Referrals are made on an "as needed basis."

Q: Who should contact SSCC?
A: Older Adults, Disabled Persons, Concerned family members, Elder law attorneys, Financial planners, Trust departments wanting information and advocacy for their clients, Discharge planners & Health professionals with concerns for patients with no family or family at a distance
