Our Process
Step 1: Assessment
Service begins with a comprehensive assessment of an individual's current circumstances and existing support system. The assessment is conducted through interviews with the client and other relevant contacts, observation (usually in the form of a visit to the client's residence) and review of additional information provided by the client or family member.
Step 2: Presentation of Care Plan
Following the assessment, a summary overview and a set of recommendations or a care plan is prepared and presented to the individual and/or caregiver. The care plan is prioritized to ensure that the most pressing needs are addressed first.
Step 3: Providing Service or Referral
There are 2 ways to proceed with care plan recommendations:
1. For families who wish to secure services through outside providers or provide care themselves, Soltis Senior Care Connections will make available a list of suggested organizations and providers to be contacted by the client or family.
2. Soltis Senior Care Connections can provide selected services while referring the family or making direct arrangements for service through third-party providers including the oversight and monitoring of such services.
Most often, it is the combination of services that creates the most effective long-term solutions and positive outcomes.
